The End-of-Life CareProgramme is Forward Living's first collaboration with Forget Thee Not. Grounded in three core elements—literacy, trust, and autonomy—the programme explores the possibility of passing away within a residential care setting. By introducing end-of-life planning and services to elderly care homes, it aims to promote palliative and end-of-life care. This empowers residents and their families to actively participate in medical and care decisions, ultimately ensuring that elderly individuals can pass away with dignity.

Target Audience
Aged 60 or above
Individuals assessed as suitable for residential care homes for the elderly and who hold a valid Hong Kong Identity Card.
Diagnosed with a terminal illness
Service users who have been diagnosed with a terminal illness.
Medical attendance during the final illness
Service users must have been attended to by a Hong Kong registered medical practitioner during their final illness within 14 days prior to their passing.
